The IHSAA has created a new consent to participate form for the 2020-21 season. Among the changes are questions about Covid-19 and new requirements for who must have a new physical to participate. ALL athletes who plan to participate in a sport during the 2020-21 school year must fill out a new consent form. If an athlete has a physical on file from the 2019-20 school year they do not need a new physical UNLESS, they had a surgical procedure performed since their last physical OR have been diagnosed with a concussion since the date of the last physical. Please follow the link below or you may find it in the Quick Links section of our website.

ELIGIBILITY REQUIREMENTS

July 1, 2010

Dear New Augusta Phoenix Athletes & Families,

In hopeful anticipation of a July 13th return to athletics, our district has created a multi-phase coaching plan that involves several precautions aimed at the safety and health of our student-athletes.  As coaches prepare to reunite with their teams in a positive, safe, and acceptable manner, we ask that you please review the following information and help us to ensure that all athletes are ready to begin on July 13th.  Select the appropriate category below based upon the status of your student-athlete during the last school year, 2019-2020.  Athletes must complete all requirements in the appropriate category before returning to play.

  1. The athlete was a member of a New Augusta High School athletic team in 2019-2020.
  • If the athlete answers “YES” to any of the questions, the athlete will need a NEW physical for the 2020-2021 school year. Click for a 2020-2021 IHSAA Physical Form and complete it in its entirety. You will need to bring the 2-sided SIGNED document AND the completed and signed Physical to the first day of conditioning.
  • If all answers are “NO,” the athlete may participate in sports at Pike High School this year under LAST YEAR’S physical (provided the Health & History & Update & Consent Form is completed and submitted).
  1. The athlete did not attend a Pike Township school last year or did not participate in a sport last year.

. Those who plan to participate in a fall sport must have the online registration process completed, and the physical form turned in, at least one day before the first practice date or the first day of tryouts. Those who do not meet this requirement cannot attend any practice or tryout session until after the requirement is met. Once the physical form is received and added to a student's online activity registration, the parent will receive an email notification, via the rschool system, of final clearance for the student's participation in summer activities/ fall sport. Those needing to submit the physical form before school is in session may: mail it to the school (ATTN: Jimmy Johnson), fax it to 317-388-7786, scan and email to , or drop it off at the front office once school buildings open back up to the public. Forms CANNOT be turned in to coaches. Prior to the start of their sports, coaches will be provided with a list of the names of those students who have completed the Activity Registration process (including submission of the physical form and who meet the academic standards described below. Those students whose names are not on the list cannot participate. Coaches will be provided updated lists as additional students register. Volleyball, soccer, and boys' tennis rosters will be finalized at the conclusion of tryouts for those sports. No one can join those teams once rosters are finalized. For cross country and football, students have until Friday, Aug. 9, to register, submit th physical form, and join.

Additionally, students must be academically eligible. All incoming 6th graders are initially academically eligible. All 7th and 8th-grade students must have achieved a 2019-2020 Quarter 4 GPA of at least 2.00 to be eligible for full participation.
