The IHSAA has created a new consent to participate form for the 2020-21 season. Among the changes are questions about Covid-19 and new requirements for who must have a new physical to participate. ALL athletes who plan to participate in a sport during the 2020-21 school year must fill out a new consent form. If an athlete has a physical on file from the 2019-20 school year they do not need a new physical UNLESS, they had a surgical procedure performed since their last physical OR have been diagnosed with a concussion since the date of the last physical.
